Yes, the judge really made that threat. And, yes, the two really came to blows. This was no a “fight.” The attorney was blindsided by the judge and physically attacked by the judge for standing up for his client’s rights. The fact that the judge was not charged just shows how social standing and someone’s position immunizes them from criminal responsibility. Had this been a member of the public or a defendant there would have been an immediate arrest.
